Certified Medical Assistant

The Corner Health Center has an immediate opening for full-time Certified Medical Assistant, 40 hours per week.
This position will work alongside clinicians, nurses, and behavioral health staff in an outpatient clinic to provide health care to adolescents, young adults, and their children.

Essential Duties and Responsibilities:

  • Vital signs, lab testing, blood draws, EKG, immunizations, and rooming patients
  • Open and close the clinic, prepare and re-stock exam rooms
  • Assist clinicians during patient exams


The Ideal Candidate:

  • At least 1 year of experience as a CMA
  • Completion of a CMA program and National Certification, or agreement to successfully obtain National Certification within 90 days of employment
  • Demonstrated proficiency and competency in taking vital signs, giving immunizations, performing blood draws, coordinating of patient flow, and other assignments as needed

Patient Services Representative

Seeking a Patient Services Representative to be part of our team and provide professional assistance to our patients and community both on the phone and at the front reception desk.

The Corner Health Center has an immediate opening for part-time Patient Services Representative (PSR). This is an hourly position, approximately 20 hours per week. This position is responsible for scheduling all patient appointments including entering and confirming information in the EMR/EHR system. Also the PSR will professionally greet and assist patients, and guests as they come into the clinic and assist in the daily operations of a busy adolescent and young adult health center.

Essential Duties and Responsibilities:

  • Schedule patient appointments.
  • Answer phones and provide telephone triage, routing calls to appropriate staff.
  • Ensure that returning patients’ records are reviewed to determine eligibility of service, and all patient information is updated.
  • Perform check out duties, including scheduling of next appointment and collecting fees.
  • Reconcile petty cash.
  • Provide clinic staff with daily information regarding patient schedule as well as updates to the schedule throughout the day.
  • Protect the confidentiality of member information in accordance with HIPAA laws and adhere to organizational policies regarding confidentiality.
  • Attend the clinic and staff meetings.

Behavioral Health Manager

About the Role

As the Behavioral Health Manager you support our mission by supervising the Behavioral Health Team, coordinating patient care with the medical team, tracking and maintaining records to support and improve continuity of care and quality of programs, and serving a client base of 16 patients per week. In addition, the BHM will work with Corner leadership and outreach staff to determine and lead community behavioral health service delivery. The successful candidate will have a passion for serving our client base along with demonstrated experience delivering therapy and supervising staff in a clinical setting.

Essential Duties and Responsibilities:

  • Supervise the Behavioral Health Team, including LLMSWs and LMSWs, with goals, expectations, one-on-one coaching, and any other team needs
  • Facilitate weekly Team Meetings and follow up with action steps, including coordinating continuing education trainings and updating the team on internal policies and general updates
  • Identify Program Improvement needs
    • Gather data and coordinate reports for program improvement needs
    • Explore and implement Therapy Groups (when applicable)
    • Assist in establishing & maintaining positive relationships between Clinical Providers and Behavioral Health Team
    • Track no-show appointments to support and improve continuity of care and treatment
  • Keep uniform records by reviewing and auditing patient files
  •  Evaluate Staff Performance
    • Monitor team’s productivity, daily task completion, review process, disciplinary actions, performance enhancement plans, and follows up with clinicians when they are not meeting expectations
    • Responsible for mentoring and delivering feedback to Social Workers on the quality of their services and meeting the time frame standards for documentation
    • Monitor team’s daily encounters and scheduling of regular appointments in order to meet productivity goals
    • Meet with team members on a regularly scheduled basis to address concerns and resolve problems; seeks support from management team on higher level matters
    • Assist with handling patient complaints related to services provided by a member of their team
  • Provide support with interventions as needed by assessments urgent patient care needs, processing patient intakes, and conducting the Warm Hand-Off Process to provide immediate care
  • Assist with maintaining/moving the behavioral health workload
    • Assign new patients to a therapist.
    • Maintain patient count for each therapist, including records on level of care (length of service, how often service is needed, etc.), discharges (when to), and follow up, with number of sessions allowed per insurance
    • Supports Social Workers with problem solving everyday issues
    • Maintain a patient load of 16 patients per week
    • Encourages Social Workers to pursue a multi-disciplinary team approach to patient care by making cross-referrals to medical services
  • Work as part of an interdisciplinary care team member for complex cases for coordination of care
  • Participate in all required meetings and collaborates with other departments as needed
  • Works on special projects, presentations, and trainings related to the Behavioral Health Team as assigned
  • Identify essential patient resources and referrals for those that experience a crisis or in need of higher level of care
  • Perform additional duties as assigned

The Ideal Candidate

  • Fully licensed Master of Social Worker licensed by the Michigan Bureau of Professional Licensing
  • Experience delivering therapy and supervising staff in a clinical setting
  • Previous experience counseling our age group

Clinical Social Worker 

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a Clinical Social Worker with In-Clinic MIHP appointments. The Clinical Social Worker functions as part of an integrated health model collaborating with medical providers to engage patients in supportive and behavioral health services. This role also includes providing intakes, discharges, and interventions as part of an interdisciplinary team in our In-Office Maternal Infant Health Program (MIHP). MIHP training will be provided. All visits take place at the Corner Health Center. The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team.

 

CORE RESPONSIBILITIES

  • Provides individual therapy to youth, young adults, and their families
  • Conducts psycho-social evaluations, develops treatments plans, graduations plans and safety plans as needed
  • Contributes to the overall health and well-being of the organization
  • Serves as Social Worker on multi-disciplinary MIHP team (includes social work, nutrition and nursing) – MIHP visits are In-Clinic; There are no home visits associated with this position
  • Maintains consistent and thorough documentation, timely records and accurate billing
  • Functions as a vital member of the Social Work team

MINIMUM REQUIREMENTS

  • State of Michigan Limited License Masters of Social Work
  • 3 to 5 years clinical experience working with adolescents and young adults, preferably, diverse and underserved populations
  • 1 year experience working with families
  • Expertise in cognitive behavioral therapy
  • Strong computer skills

PREFERRED SKILLS

  • State license LMSW
  • Proficiency in electronic health records is preferred
  • Supervisory experience a plus

Community Health Educator

Position Description 

The Corner Health Center has an immediate opening for full-time Community Health Educator. This multifaceted 40 hour per week position coordinates community health outreach programs and materials, with emphasis on promoting the programs and services of Corner Health Center to all youth ages 12 – 25 years old. The Community Health Educator also facilitates health discussions for our Theatre Troupe as preparation for peer to peer health performances on topics such as depression, HIV/STI prevention, healthy relationships, bullying, and teen-parent communication. In coordination with the Theatre Troupe Director, the Community Health Educator will coordinate auditions, recruitment, and performances of the troupe, as well as provide support, resources, and training in health education. Additionally, the CHE will attend health-specific community meetings, coordinate the Corner’s outreach programming, and assist with grants dealing with the above mentioned areas.

Core Responsibilities

  • Schedule and administer all activities of the Theatre Troupe, including training, rehearsals, and performances
  • Provide health education training to actors in troupe, Corner staff, and at relevant community events
  • Assist with grant proposals and Theatre Troupe program budget; review and implement contracts; oversee expenditures; develop and coordinate evaluation; and report to funding agencies and foundations
  • Develop and review health education materials for Corner outreach efforts
  • Attend health specific community task force and coordinated meetings
  • Coordinate Corner health outreach programming
  • Provide staff support to Youth Leadership Council
  • Create and implement year-round Health Youth Healthy Futures initiatives in person and virtually including facilitating the Summer Nutrition program in collaboration with community partners
