POSITION DESCRIPTION

The Corner Health Center has an immediate opening for part-time Executive Assistant. This individual will provide support to the Executive Director and the agency leadership team. This is an hourly, part-time position (20-30 hours per week, including some evening and weekend hours).  This person will report directly to the Executive Director.

CORE RESPONSIBILITIES

  • Provide full administrative support to the Executive Director.
  • Maintain the Executive Director’s calendar, responds to doodle polls and calendar requests.
  • Perform word-processing and typing for senior leadership team as assigned; performs copying, mailing, and other clerical tasks.
  • Assist in collating and mailing of proposals and necessary reports.
  • Attend assigned meetings and prepares minutes for distribution; prepares mailings to Board and other committee members; maintains and updates accurate address lists for Board and various other agency-associated committees.
  • Collaborate with Associate Director in completion of monthly utilization reports.
  • Update and maintain computer files of forms, letters, time sheets, etc. for agency.
  • Reconcile agency credit card receipts as assigned.
  • Help to prepare materials for marketing and fundraising mailings, including some desk-top publishing.
  • Help to prepare for fundraising events and assists in fundraising mailings of brochures, flyers and newsletter.
  • Assist in coordination of agency publicity and community relations activities; assist in creating and staffing displays for special agency public relations events.
  • Assist in coordinating volunteer activities for special events.

REQUIREMENTS

  • Associates Degree or equivalent
  • Minimum of 2-5 years of administrative support
  • Excellent written, oral, and interpersonal communication skills.
  • Strong external networking, relationship and customer service skills.
  • Proficiency in Microsoft Excel, Word, Outlook, and PowerPoint.
  • Detail-oriented and highly organized.
  • Ability to keep sensitive and critical information absolutely confidential.
  • Ability to work well as part of a team and independently.
  • Appreciation for the agency mission.
  • Maintains confidentiality.

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for part-time Front Desk Receptionist. This is an hourly, part-time position (30 hours per week, including some evening hours). This position will professionally greet and assist patients, providers, residents, staff, and guests; enter and confirm information within an EMR/EHR system; and assist in the daily operations of a busy adolescent and young adult health center.

CORE RESPONSIBILITIES

  • Answer phones and provide telephone triage, routing calls to appropriate staff
  • Make patient appointments as needed.
  • Ensure that returning patients’ records are reviewed to determine eligibility of service, and all patient information is updated.
  • Assist patients in applying for health insurance and filling out forms.
  • Perform check out duties, including scheduling of next appointment and collecting fees.
  • Reconcile petty cash.
  • Provide food coordination in the Corner Store and Food Gatherers ordering.
  • Provide clinic staff with daily information regarding patient schedule as well as updates to the schedule throughout the day.
  • Protect the confidentiality of member information in accordance with HIPAA laws and adhere to organizational policies regarding confidentiality.
  • Attend clinic and staff meetings.

REQUIREMENTS

  • High School Diploma or equivalent.
  • Experience with EMR/EHR
  • Able to work flexible hours.
  • Proven ability to work with disenfranchised and oppressed populations.
  • High level of professionalism.
  • Strong customer service skills.
  • Excellent written, oral, and interpersonal communication skills.

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for full-time Associate Director with a working knowledge of nonprofit management, ideally within a healthcare setting. This individual will work very closely with the Executive Director and the agency leadership team, and will oversee all clinical activities for the organization including primary care and behavioral health services and programs. This is an exempt, full time position (40 hours per week, including some evening and weekend hours).  This person will report directly to the Executive Director. This is an ideal position for someone with several years of progressive management experience, with a passion for helping lead all aspects of a strong, local community organization, and with a passion for youth and health. 

The Associate Director is responsible for agency operations overall, with a specific focus on clinic operations. In the absence of the Executive Director, the Associate Director acts on his/her behalf and is empowered to make decisions as appropriate, reporting to the Board of Directors as required to carry out the Executive Director role. Responsibilities include oversight of integrated primary care and behavioral health programs; agency-wide program development, implementation and evaluation; grant writing and reporting; strategic planning; building community partnerships with health and youth serving organizations; developing fiscal year operating budgets; and partnering with leadership team to analyze and increase patient capacity and agency revenue. 

 

CORE RESPONSIBILITIES

Medical Team:

  • Provide oversight to clinical services in partnership with Medical Director, to ensure policies and procedures result in an efficient and effective clinic, including ensuring maintenance and adherence to all policies and procedures that include but are not limited to HIPAA, OSHA, CLIA and MDHHS regulations, operational and clinic policies, laboratory and patient care. Acts as the HIPAA compliance officer.
  • Provide direct supervision to Clinic Manager and Behavioral Health Manager (who in turn supervise all clinic staff).
  • Track and provide resolution to patient complaints.
  • Facilitate Clinic and Quality Assurance meetings.

Administrative

  • Program development, implementation and evaluation in line with agency priorities and funding needs.
  • Evaluate, monitor and report on agency performance to Board and funders as assigned.
  • Participate in annual budget development and monitor budgetary compliance for areas of responsibility including developing and implementing revenue enhancement and expense management strategies.
  • Assist in the development and implementation of the agency annual strategic plan
  • Attend leadership team, Board, and other meetings as needed.
  • Attend appropriate community meetings as a Corner Health Center representative.
  • Oversee the revision and proper implementation of written Operating Policies and Procedures, including personnel policies, and operating policies driven by best practices in youth services.
  • Work with Executive Director and Director of Finance to ensure agency spending is in line with Board-approved budget, and seek approval for changes or exceptions to budget when necessary.
  • Facilitate the agency’s Leadership Team to identify best practice standards in work with high risk youth and families, and make improvements and modifications.
  • Develop maintenance plan for all facilities which includes priority levels, time frames, and estimated costs.
  • Oversee the acquisition of contractors, materials, permits, and inspections, as pertinent, for needed repairs and rehabilitation projects.
  • Serve as a liaison between staff and IT support contractors, including assisting IT contractors integrate new hardware, software, and systems.
  • Oversee and participate in recruiting, interviewing, and hiring of new professional direct services staff.   Develop and maintain personnel files and job descriptions for all professional direct service staff.
  • Ensure that all direct service staff have opportunities for training and professional development in line with needs of the agency, budget, and contractual obligations.
  • Provide written performance evaluations to all supervisory staff completing their orientation period and to all others on an annual basis.

REQUIREMENTS

  • Master’s Degree in related field required.
  • 5+ years of increasing responsibility in non-profit management in a health care organization preferred.
  • Understanding of the health care industry.
  • Excellent written and oral communication skills, including the ability to effectively interact with a diverse variety of stakeholders and constituents.
  • Strong external networking, relationship and customer service skills.
  • Proven ability to plan, monitor and grow revenue streams.
  • Experience creating annual operating budget and proactively managing budget variances.
  • Proficiency in Microsoft Excel, Word, Outlook, and PowerPoint.
  • Experience with database management required; experience with Electronic Health Records preferred.
  • Detail-oriented and highly organized.
  • Ability to keep sensitive and critical information absolutely confidential.

This position is currently 40 hours per week, including some evenings in our specialized adolescent health care clinic. This is a 3-year grant funded position with potential for continuation. The Clinical Social Worker functions as part of an integrated health team collaborating with medical providers to engage patients in supportive and behavioral health services. The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team.

CORE RESPONSIBLITIES

  • Provides individual therapy to youth, young adults, and their families
  • Conducts psycho-social evaluations, develops treatment plans, graduation/termination plans and safety plans as needed
  • Contributes to the overall health and well-being of the organization
  • Maintains consistent and thorough documentation, timely records and accurate billing
  • Functions as a vital member of the Social Work team and the integrated health team.

REQUIREMENTS

  • State license LMSW (LLMSWs will not be considered for this position)
  • 3 to 5 years clinical experience working with adolescents and young adults, preferably, diverse and underserved populations
  • Minimum 1 year experience working with families
  • Expertise in cognitive behavioral therapy, motivational interviewing, and trauma-focused therapy.
  • Strong computer skills

PREFERRED SKILLS

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a full-time Clinical Social Worker with In-Clinic MIHP appointments. This position is currently 32 hours per week, including some evenings in our specialized adolescent health care clinic. The Clinical Social Worker functions as part of an integrated health team collaborating with medical providers to engage patients in supportive and behavioral health services. This role also includes providing intakes, discharges, and interventions as part of an interdisciplinary team in our In-Clinic Maternal Infant Health Program (MIHP).  [MIHP training will be provided. All visits take place at the Corner Health Center.] The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team. Schedule is flexible, but must include Monday and Wednesday evenings, and Tuesday and Thursday afternoons.

CORE RESPONSIBLITIES
