ORGANIZATIONAL SUMMARY

The Corner Health Center is Michigan's first, largest and most comprehensive adolescent and young adult health center. The Corner serves young people ages 12 through 25 year and has nearly 40 years of experience providing care to individuals with complex needs. The Corner provides integrated primary and mental health care in a culture of dignity and respect.

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a full-time Development Director. This is a 40 hours per week position. The Development Director will develop, implement, and oversee the fundraising initiatives for the organization. With the Executive Director, Board Fundraising Committee, and staff, the Director will meet financial goals and timetables, write grants to support clinic operations and special programs, organize events, and coordinate donor communications and appeals.  

CORE RESPONSIBLITIES

  • Develop and implement a comprehensive development and public relations strategy that meets the agency’s needs
  • Meet specific financial goals and timetables developed in conjunction with the Executive Director and Board Fundraising Committee
  • Recommend a budget to support the public relations and development plan
  • Write grants to support the medical clinic and special projects
  • Manage relationships with funders, including creating necessary reports
  • Communicate with donors through mailings and newsletters, and manage annual appeals
  • Organize, market, implement donor events
  • Oversee the donor database to ensure accuracy and growth
  • Attend community events to develop relationships and foster visibility within the community

REQUIREMENTS

  • Bachelor of Arts
  • Minimum of 3 years fundraising and marketing experience
  • Excellent verbal and written communication skills
  • Ability to work as part of a team
  • Ability to work flexible hours, including some evenings and weekends

PREFERRED SKILLS

  • Experience/service in a community health/medical setting

Corner Health Center seeks full-time Nurse Clinical Care Coordinator

The Corner provides integrated primary and mental health care in a culture of dignity and respect.

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a full-time Nurse Clinical Care Coordinator. This position is 40 hours per week, including some evenings in our specialized adolescent health care clinic. The Nurse Clinical Care Coordinator provides comprehensive case management to our obstetrics and gynecology, transgender, psychiatry, (in-house) MIHP, and pediatric patients through collaboration with our multidisciplinary team of physicians, nurses, MAs, social workers, and program coordinators. The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multidisciplinary team.

CORE RESPONSIBLITIES

  • Plans, implements, manages, and coordinates comprehensive plans of care for a diverse patient population
  • Coordinates with a multidisciplinary team to provide comprehensive information and care
  • Manages labs, reports labs to clinicians, and follows up as necessary
  • Maintains patient databases, creating reports for funders, venders, and donors
  • Attends daily, weekly, and monthly meetings
  • Triages patient phone calls as needed
  • Actively participates in the referral processes, including eligibility verification, compliance with medical necessity guidelines and outcomes, and ensuring appropriate information is entered into all relevant data systems

REQUIREMENTS

  • Bachelor degree in Nursing
  • Current Michigan RN License
  • Strong computer skills

PREFERRED SKILLS

  • 2+ years of experience preferred
  • Proficiency in electronic health records
  • Experience/service in a community health/ medical setting

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for full-time Certified Medical Assistant. This position is 40 hours per week and will include evening hours in our specialized adolescent health care clinic. This position will work alongside clinicians, nurses and behavioral health staff in an outpatient clinic to provide health care to adolescents and young adults. No Certified Nursing Assistants will be considered for this position.

CORE RESPONSIBILITIES

  • Maintain proficiency and competency in performing the tasks of taking vital signs, immunizations, coordination of patient flow, and other assignments as needed.
  • Open and close the clinic, and prepare and restock rooms
  • Assist clinicians during patient exams
  • Accurately perform lab tests
  • Maintain clinic inventory

 

REQUIREMENTS

  • 1-2 years of experience as MA (Immunizations, blood-draws, taking vitals, etc.)
  • Proven ability to work with disenfranchised and oppressed populations
  • High level of professionalism
  • Strong customer service skills
  • Inventory management experience
  • Excellent communication and writing skills
  • Willingness to work with a team
  • Working knowledge of Electronic Medical Records
  • Strong computer skills

 

PREFERRED SKILLS

  • Proficiency in electronic health records is preferred

POSITION DESCRIPTION

The Corner Health Center Clinic Manager will serve in a critical leadership role managing and maintaining day to day clinic operations of an adolescent health clinic. The Clinic Manager will lead a team of outstanding clinicians, all devoted to excellent patient care. This position requires an experienced clinician with outstanding leadership and organizational skills. The Clinic Manager will partner with the center's staff to ensure the highest standard of integrated healthcare delivery for patients. The ideal candidate is a high energy problem solver and effective team-builder.

 

CORE RESPONSIBILITIES

  • Establishes policies, protocols, procedures, and other guidelines for clinical organization, in collaboration with the Medical Director
  • Facilitates efficient and effective clinic operations and adherence to policies and procedures
  • Executes effective and optimal patient care management and service delivery
  • Provides on-site supervision in the clinic
    • Identifies and implements best practices, and addresses opportunities for clinical and process improvements
    • Provides oversight for clinicians to maintain up-to-date and accurate records/billing
    • Ensures clinical compliance with all state practices/rules/regulations and competencies
    • Contributes to the overall health and well-being of the organization
    • Provides oversight for the vaccination administration program
    • Maintains consistent and thorough documentation, timely records and accurate billing
    • Functions as a critical member of the leadership team
    • Provides clinical care as back-up, as needed

REQUIREMENTS

  • State licensed RN
  • 3 to 5 years in a supervisory role
  • 3 to 5 years clinical experience working with adolescents and young adults, preferably, diverse and underserved populations
  • Minimum 1 year experience working with families
  • Expertise in organization and management
  • Strong computer skills

 

PREFERRED SKILLS

  • MSN preferred
  • Proficiency in electronic health records is preferred
  • Spanish-language speaker a plus
  • Experience/service in a community health/ medical setting
  • Experience with OB patients a plus

Corner Health Center seeks part-time Clinical Social Worker with In-Clinic MIHP

*There are no at-home MIHP visits associated with this position

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a part-time Clinical Social Worker with In-Clinic MIPH appointments. This position is currently 20 hours per week, including some evenings in our specialized adolescent health care clinic. The Clinical Social Worker functions as part of an integrated health model collaborating with medical providers to engage patients in supportive and behavioral health services. This role also includes providing intakes, discharges, and interventions as part of an interdisciplinary team in our In-Office Maternal Infant Health Program (MIHP).  [MIHP training will be provided. All visits take place at the Corner Health Center.] The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team.

CORE RESPONSIBLITIES

  • Provides individual therapy to youth, young adults, and their families
  • Conducts psycho-social evaluations, develops treatments plans, graduations plans and safety plans as needed
  • Contributes to the overall health and well-being of the organization
  • Serves as Social Worker on multi-disciplinary MIHP team (includes social work, nutrition and nursing) – MIHP visits are In-Clinic; There are no home visits associated with this position
  • Maintains consistent and thorough documentation, timely records and accurate billing
  • Functions as a vital member of the Social Work team

REQUIREMENTS

  • State license LMSW (LLMSWs will not be considered for this position)
  • 3 to 5 years clinical experience working with adolescents and young adults, preferably, diverse and underserved populations
  • Minimum 1 year experience working with families
  • Expertise in cognitive behavioral therapy
  • Strong computer skills
