POSITION DESCRIPTION

The Corner Health Center seeks a full-time (40 hours per week) Community Outreach / Youth Development Coordinator to oversee the youth-centered programs of a nonprofit health center for young adults. The Corner is committed to youth voice and active involvement throughout our programming and services. This position facilitates programs including the Youth Leadership Council and Community Advisory Council; supervises the drama-based, peer education Theatre Troupe’s Health Educator and Theatre Director; and works closely with community partners to ensure widespread knowledge of the Corner’s services and to facilitate collaborative efforts.

The candidate must enjoy working with adolescents and young adults, possess excellent oral and written communication skills, enjoy teamwork and working on an interdisciplinary team. Experience working with adolescents and families is strongly preferred. Bachelor’s Degree required, Master’s Degree preferred in social work, public health or a related field. Prior experience providing supervision strongly preferred.

CORE RESPONSIBILITIES

Youth Development:

  • Assess programs and services to ensure they meet standards of high-quality, best practices in positive youth development.
  • Supervise, train and evaluate youth development programs and staff
  • Serve as staff advisor to youth leaders on youth-driven programming, youth on the Board of Directors and ensure youth Board members are appropriately engaged, feel included, and understand all the necessary information.
  • Participate in writing proposals, reporting, and implementing grants to ensure sustainability for youth development programs.
  • Ensure youth voice informs Corner services and programs including participation in employee interviews as appropriate, providing feedback on printed materials, and involvement in other programmatic needs.
  • Identify and participate in appropriate community meetings, task forces, and coalitions.

Outreach:

  • Promote and build relationships with community partners, referral organizations, parents and youth to expand the reach of the Corner’s mission.
  • Work with Development Director and other relevant staff to plan, implement and evaluate the strategy for all printed and online communications (including the Corner’s website, social media, e-newsletter, outreach materials, and public relations messages) to consistently articulate the Corner Health Center’s mission and brand.
  • Coordinate participation in relevant outreach events and distribution of outreach materials.
  • In partnership with the Development Director, support the Board Community Engagement and Resource Development committee to promote Board engagement in communications, marketing, outreach, and development.

Other Duties:

  • Collaborate with staff to plan, implement and evaluate Corner community events.
  • Plan and facilitate Community Advisory Council meetings.
  • Partner with Associate Director to implement patient satisfaction survey twice each year.
  • Participate in annual budget development and monitor budgetary compliance for areas of responsibility.
  • Assist with gathering data from youth programming for internal and external reporting (e.g. strategic plan, Board meetings, grantors, etc.).
  • Attend Board meetings as needed.

MINIMUM REQUIREMENTS

  • Bachelor’s degree or Master's degree in Public Health, Social Work, or related field.
  • Ability to work and connect with diverse communities, people and partners, and a commitment to promoting inclusion in all practices and efforts.
  • Strong written and verbal communication and public presentation skills.
  • Ability to plan, organize and think strategically.

The Corner Health Center has an immediate opening for a part-time Clinical Social Worker. This position is currently 20 hours per week, including some evenings in our specialized adolescent health care clinic. The Clinical Social Worker functions as part of an integrated health model collaborating with medical providers to engage patients in supportive and behavioral health services. The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team. Schedule is flexible but must include Monday and Wednesday evenings, and Tuesday afternoons.

  • Provides individual therapy to youth, young adults, and their families
  • Conducts psycho-social evaluations, develops treatments plans, graduations plans, and safety plans as needed

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for a part-time Clinical Social Worker with In-Clinic MIPH appointments. This position is currently 32 hours per week, with 5-10 hours being part of the In-house MIHP program, and including some evenings in our specialized adolescent health care clinic. The Clinical Social Worker functions as part of an integrated health model collaborating with medical providers to engage patients in supportive and behavioral health services. This role also includes providing intakes, discharges, and interventions as part of an interdisciplinary team in our In-Office Maternal Infant Health Program (MIHP).  [MIHP training will be provided. All visits take place at the Corner Health Center.] The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multi-disciplinary team. Schedule is flexible but must include Monday and Wednesday evenings, and Tuesday and Thursday afternoons.

CORE RESPONSIBILITIES

  • Provides individual therapy to youth, young adults, and their families

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for full-time Community Health Educator. This is a multifaceted 40 hours per week position centering on the coordination of the Corner Health Center Theater Troupe -- a peer health education program that uses theatre as a tool to teach young adults how to be healthy – as well as the coordination of community health outreach programs and materials. Troupe members help write and perform shows all over Southeast Michigan on topics such as depression, HIV/STI prevention, healthy relationships, bullying, and teen-parent communication. In coordination with the Theatre Troupe Director, the Community Health Educator will coordinate auditions, recruitment, and performances of the troupe, as well as provide support, resources, and training in health education. Additionally, the CHE will attend health-specific community meetings, coordinate the Corner’s outreach programming, and assist with grants dealing with the above-mentioned areas.

CORE RESPONSIBILITIES

  • Schedules and administers all activities of the Theatre Troupe, including training, rehearsals, and performances
  • Provides health education training to actors in troupe, Corner staff, and at relevant community events
  • Prepares and distributes written health education materials to Troupe members; disseminates educational and Corner materials to performance audience members
  • Works with Theater Director by providing health materials and research for development of Theatre Troupe plays
  • Schedules auditions/recruitment activities, performances, and workshops
  • Maintains and updates training materials in Theatre Troupe manual
  • Provides support, resources, and guidance for Theatre Troupe members as needed
  • Facilitates health discussions at performances
  • Ensures that evaluation data are collected; maintains accurate records of such data; facilitates entering data into computer as needed; works cooperatively with evaluation and reporting team
  • Assists with grant proposals and Theatre Troupe program budget; reviews and implements contracts; oversees expenditures; develops and coordinates evaluation, and reports to funding agencies and foundations
  • Works collaboratively with the theatre instructor
  • Develops and reviews health education materials for Corner outreach efforts
  • Attends health specific community task force and coordinated meetings
  • Coordinates Corner health outreach programming
  • Provides staff support to Youth Leadership Council

REQUIREMENTS

  • Bachelor’s Degree in Public Health, Social Work, Health Sciences, or related field
  • Excellent organizational skills and ability to manage multiple priorities simultaneously
  • Excellent written and oral communication skills, including the ability to effectively interact with a diverse variety of stakeholders and constituents.

The Corner Health Center has an immediate opening for a full-time Nurse Clinical Care Coordinator for our fast-paced, team-oriented, specialized adolescent health care clinic. This is a salaried position at 40 hours per week, including some evenings. The Nurse Clinical Care Coordinator provides comprehensive case management and clinical care to our obstetrics and gynecology, transgender, psychiatry, MIHP (in-house), and pediatric patients through collaboration with our multidisciplinary team of physicians, nurses, MAs, social workers, and program coordinators. The candidate must like working with young people, possess excellent communication skills, enjoy teamwork and working with a multidisciplinary team. This is an ideal position for anyone with an all-hands-on-deck attitude who wants to directly engage with patients’ lives, make a difference in the community, and has a good sense of humor.

CORE RESPONSIBILITIES

  • Triages patient direct and indirect care, including blood draws, immunizations, labs, and phone calls
  • Plans, implements, manages, and coordinates comprehensive plans of care for a diverse patient population including obstetrics and gynecology, transgender, pediatric, and Mother Infant Health Program (in-clinic) patients
  • Manages prescription refill hotline, coordinating patient, provider, and pharmacy
  • Coordinates with a multidisciplinary team to provide comprehensive information and care
  • Manages labs, reports labs to clinicians, and follows up as necessary
  • Maintains patient databases, creating reports for funders, vendors, and donors
  • Attends daily, weekly, and monthly meetings
  • Actively participates in the referral processes, including eligibility verification, compliance with medical necessity guidelines and outcomes, and ensuring appropriate information is entered into all relevant data systems

REQUIREMENTS

  • Bachelor degree in Nursing

POSITION DESCRIPTION

The Corner Health Center has an immediate opening for full-time Associate Director with a working knowledge of nonprofit management, ideally within a healthcare setting. This individual will work very closely with the Executive Director and the agency leadership team, and will oversee all clinical activities for the organization including primary care and behavioral health services and programs. This is an exempt, full time position (40 hours per week, including some evening and weekend hours).  This person will report directly to the Executive Director. This is an ideal position for someone with several years of progressive management experience, with a passion for helping lead all aspects of a strong, local community organization, and with a passion for youth and health. 

The Associate Director is responsible for agency operations overall, with a specific focus on clinic operations. In the absence of the Executive Director, the Associate Director acts on his/her behalf and is empowered to make decisions as appropriate, reporting to the Board of Directors as required to carry out the Executive Director role. Responsibilities include oversight of integrated primary care and behavioral health programs; agency-wide program development, implementation, and evaluation; grant writing and reporting; strategic planning; building community partnerships with health and youth-serving organizations; developing fiscal year operating budgets; and partnering with leadership team to analyze and increase patient capacity and agency revenue. 

 

CORE RESPONSIBILITIES

Medical Team:

  • Provide oversight to clinical services in partnership with Medical Director, to ensure policies and procedures result in an efficient and effective clinic, including ensuring maintenance and adherence to all policies and procedures that include but are not limited to HIPAA, OSHA, CLIA and MDHHS regulations, operational and clinic policies, laboratory and patient care. Acts as the HIPAA compliance officer.
  • Provide direct supervision to Clinic Manager and Behavioral Health Manager (who in turn supervise all clinic staff).
  • Track and provide resolution to patient complaints.
  • Facilitate Clinic and Quality Assurance meetings.

Administrative

  • Program development, implementation, and evaluation in line with agency priorities and funding needs.
  • Evaluate, monitor and report on agency performance to Board and funders as assigned.
  • Participate in annual budget development and monitor budgetary compliance for areas of responsibility including developing and implementing revenue enhancement and expense management strategies.
  • Assist in the development and implementation of the agency annual strategic plan
  • Attend leadership team, Board, and other meetings as needed.
  • Attend appropriate community meetings as a Corner Health Center representative.
  • Oversee the revision and proper implementation of written Operating Policies and Procedures, including personnel policies, and operating policies driven by best practices in youth services.
  • Work with Executive Director and Director of Finance to ensure agency spending is in line with Board-approved budget, and seek approval for changes or exceptions to budget when necessary.
  • Facilitate the agency’s Leadership Team to identify best practice standards in work with high-risk youth and families, and make improvements and modifications.
  • Develop a maintenance plan for all facilities which includes priority levels, timeframes, and estimated costs.
  • Oversee the acquisition of contractors, materials, permits, and inspections, as pertinent, for needed repairs and rehabilitation projects.
  • Serve as a liaison between staff and IT support contractors, including assisting IT contractors integrate new hardware, software, and systems.
  • Oversee and participate in recruiting, interviewing, and hiring of new professional direct services staff.   Develop and maintain personnel files and job descriptions for all professional direct service staff.
  • Ensure that all direct service staff has opportunities for training and professional development in line with the needs of the agency, budget, and contractual obligations.
  • Provide written performance evaluations to all supervisory staff completing their orientation period and to all others on an annual basis.

REQUIREMENTS

  • Master’s Degree in related field required.
  • 5+ years of increasing responsibility in non-profit management in a health care organization preferred.
  • Understanding of the health care industry.
  • Excellent written and oral communication skills, including the ability to effectively interact with a diverse variety of stakeholders and constituents.
  • Strong external networking, relationship, and customer service skills.
  • Proven ability to plan, monitor and grow revenue streams.
  • Experience creating annual operating budget and proactively managing budget variances.
